Adoption Agreement
Listed below are the terms and conditions of the MaPaw Adoption Agreement.
Background
MaPaw Siberian Husky Rescue & Referral Service, Inc. has become the owner of the dog(s) described above from the named place of rescue and has paid all required expenses or donations.
MaPaw Siberian Husky Rescue & Referral Service, Inc. has not undertaken any investigation with regard to the history or physical condition of the dog(s).
MaPaw Siberian Husky Rescue & Referral Service, Inc. has, in good faith, attempted to find a suitable home for the dog(s) in an effort to save the dog’s life.
The adopter has agreed to adopt the dog(s) from MaPaw Siberian Husky Rescue & Referral Service Inc. upon the terms and conditions set forth in this agreement, and based upon the adopter’s own observations and inspection.
Now, therefore, the parties intending to be legally bound, hereby agree as follows:
The adopter agrees to an adoption fee of $_______________. The adopter agrees to give the Siberian Husky one (1) to three (3) months to adjust to new surroundings, to bond to family and to fit in as a family member.
The adopter agrees to keep and maintain the dog(s) in a safe, healthy, friendly and proper environment.
If at any time, the adopter does not wish to keep the dog(s), the adopter shall first notify MaPaw Siberian Husky Rescue & Referral Service, Inc. giving MaPaw reasonable and first opportunity to regain ownership and possession of the dog(s) at no cost or expense to MaPaw. The adopter will make all necessary arrangements to return the dog to MaPaw Siberian Husky Rescue & Referral Service, Inc. UNDER NO CIRCUMSTANCE IS THE DOG(S) TO BE SOLD, GIVEN, TRANSFERRED OR IN ANY WAY DISPOSED OF WITHOUT WRITTEN NOTIFICATION TO MAPAW SIBERIAN HUSKY RESCUE & REFERRAL SERVICE, INC. AND RIGHT OF FIRST REFUSAL BY MAPAW.
The adopter agrees to notify MaPaw Siberian Husky Rescue & Referral Service, Inc. if the dog(s) is lost or stolen and to make every effort to locate the dog(s).
The dog(s) obtained by (the adopter), shall not be used for stud service or as a brood bitch and if the dog(s) is not neutered/spayed at the time of the adoption, the dog(s) will be neutered/spayed within 60 days following the adoption by a recognized veterinarian. The adopter agrees to this condition, understanding and acknowledging the desire and intent to limit the number of puppies and unwanted dogs that may be destroyed or bred in such a way that may adversely affect the health and future of the Siberian Husky breed.
The adopter agrees not to allow the dog(s) to be used in animal research, dog(s) fighting, as a guard dog(s), attack dog(s) or protection dog(s) for any agency, firm, company or organization.
The adopter agrees to license, put and keep personal ID tags on the dog(s) with a strong collar preferably a rolled leather one and never leave a choke chain on the dog(s) when unattended. Choke collars are properly used for walking and obedience training ONLY.
Pennsylvania Law requires that your dog(s) always be leashed when off the owner’s property. Heavy fines can be levied if your dog(s) “runs free”.
MaPaw Siberian Husky Rescue & Referral Service, Inc. has made no warranties or guarantees, express or implied, with regard to the physical condition of the dog(s), its background or its suitability or compatibility.
MaPaw Siberian Husky Rescue & Referral Service, Inc. shall not be responsible for any and all damages, costs or expense incurred by the adopter resulting from this placement, including but not limited to, damage or injury to persons or property by the dog(s), and in this connection, the adopter releases, indemnifies, and holds MaPaw harmless from any and all liability of any and every nature and cause directly or indirectly relating to this placement. The adopter has read and understood the information package given to him/her detailing all adoption responsibilities.
